Rework coa entry form: Separate each description act into its one subpage

Separate each description act into its one subpage instead of using composide forms.

• add a table and semantic query to rsp:RepresentationForm to list description acts
• add new html form for each description act. This should continue to use the subforms for blazoning (and -- for now -- for attribution)
• finish #97 (closed)
• create additional issue with priority 2: blazoning acts and attribution/identification acts should be separated -> this is now easier to implement since on each rsp:RepresentationForm there can be a table for all blazoning acts and another table for all attribution acts
• create additional issue to add a delete button to each line of each description act table row
